Boarding Appointment Form

Thank you for requesting an appointment with Luv-N-Care Animal Hospital, P.A.. We look forward to meeting all of your veterinary needs. Please remember that your appointment is not final until you receive confirmation from our staff.

Please note: You may be required to sign release forms in our office, as well as provide us with a copy of your pet's vaccination certificates. No reservations are final until you receive confirmation from our staff.

Boarding Policies:
We require that all pets be picked up between 8:00 AM and 6:00 PM Monday through Friday and between 8:00 AM and 3:00 PM Saturday. Boarding fees are charged by the number of nights the pet spends at the kennel. All belongings with the client's name clearly marked will be returned as soon as available. The Kennel is not responsible for any personal belongings left with the pet that are not clearly marked. If some belongings become soiled during the stay, we may be in the process of washing them and will therefore return them when they are clean unless otherwise agreed upon with the owner.

For your pet's health, all pets are required to be current on all vaccines. The Bordetella vaccine needs to be current within 6 months of boarding. Proof of vaccinations is required. If you are a new client, it will shorten the check-in time if you could procure vaccine records in advance.

Your pet must be free from all external and internal parasites upon entering our establishment. We require fecal exams for pets not taking a heartworm preventative.

Reasonable precautions will be followed to prevent any unlikely event of injury, escape or passing away of the pet. The clinic and it's staff will not be held liable for any problems that develop (most commonly fleas & ticks), provided reasonable care and precautions are followed.

It will be understood by the client that any problem that arises with the pet will be attended by the staff veterinarian and staff and the owner of the pet will be responsible for all expenses for the necessary treatment.
